Manuka honey costs a lot more than regular honey, so it's a fair question: what's actually different about it, and does the evidence back up the reputation?
Manuka honey is made by bees foraging on the manuka bush (Leptospermum scoparium), native to New Zealand and parts of Australia. The nectar of the manuka flower contains a compound called dihydroxyacetone, which converts into methylglyoxal (MGO) as the honey matures. MGO is what gives manuka honey its distinctive, and much higher, antibacterial activity compared with regular honey.
It's worth correcting a common mix-up here: regular honey's antibacterial activity mostly comes from hydrogen peroxide, produced by an enzyme bees add called glucose oxidase. Manuka honey doesn't simply "supercharge" that same enzyme, MGO is a separate, additional compound that regular honey doesn't contain in any meaningful amount. That's the real reason manuka honey is graded and priced differently, not vague "extra potency."
Laboratory studies have found that MGO and manuka honey can inhibit the growth of several bacteria, including H. pylori, MRSA, E. coli and C. difficile, in test-tube conditions. That's genuine, published research, but it's lab evidence of antibacterial activity, not proof that eating a spoonful of honey treats an infection in your body. We're not going to blur that distinction.
Where the evidence is strongest, and where manuka honey is genuinely used in modern medicine, is wound care. Medical-grade, sterilised manuka honey is formulated into licensed wound dressings used in the NHS and other health services, including for some surgical and chronic wounds. That's a real, well-documented use, but it's a specific, sterilised medical product, not the same as the jar of honey on your breakfast table, and it isn't something to try replicating at home on a wound.
New Zealand's seasons are the reverse of the UK's, so manuka bushes flower in the New Zealand summer, and harvest typically runs from around January to March, once the bees have finished foraging on the brief manuka bloom. Much of it is produced in remote areas away from heavy industry, which is a genuine, practical reason it's often purer than honey produced closer to agricultural or urban areas, though that's a separate point from organic certification. Not being certified organic doesn't automatically make a honey less pure, and being remote doesn't automatically make it certified organic either. They're two different things.
Honeybees keep their hive at a remarkably stable 33 to 36°C. Heating honey much above that, generally cited around 40°C, starts to break down some of its natural enzymes, which is why "raw" honey is defined by not being heated beyond roughly the hive's own temperature. It's a genuine, meaningful distinction, not just a label.
Manuka honey's reputation isn't hype, it comes from a genuine, well-researched compound that regular honey largely doesn't have. Whether that's worth the price difference for you comes down to what you're using it for, a spoonful in your tea is a different proposition to a clinical wound dressing.
